MMC Application or Common Application & Common Application Supplement You may print the application form directly, fill out, and send it using regular mail to the Office of Admission.

Recommendation forms Print the recommendation forms and give them to your teacher, counselor and/or employer to complete.

Judicial Record Form To be completed by transfer or non-degree students who attended a previous college or university. Print and give the form to your Dean of Students to complete and mail to us.

Academic Access (fee-based Learning Disability program) To be completed by students applying to the Program for Academic Access. Specific documentation of a learning disability is required along with this application. Students must be admitted to MMC prior to being considered for this program.

Homeschool Student Information Form To be completed by students who have been homeschooled at any time during high school and submitted with their homeschool transcript.

Spring Deferral Application Print and submit along with an official final high school transcript and most recent official college transcripts (if applicable). If attending college during the fall, be sure to submit those official transcripts once courses are complete. Students may apply for a deferral from the fall to spring semester only. Keep in mind that your current academic record will be reviewed for consideration of deferral acceptance and scholarship eligibility.
